Discover a flexible role with the stability of making a difference as a full-time Signifier! *Signify Health, part of CVS Health®, is hiring full-time board-certified nurse practitioners *to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.
*In this role, you will:*
* Provide availability to complete 160 in-home visits per month
* Shifts available Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.* Conduct pre-scheduled in-home visits with health plan members
* Conduct video visits as needed
* Malpractice insurance coverage (during Signify Health-related activities)
* Maintain schedule flexibility
* *NY State License Required*
During our In-Home Health Evaluations (and other associated follow-up visits), you’ll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.
The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.
